Overview
Specialty Add-On Programs are available to active members age 18 and older who need additional support beyond routine primary care. These programs include focused provider oversight, program-related basic lab monitoring, treatment planning, safety review, and ongoing care coordination when clinically appropriate.
A $99 Specialty Program Evaluation is required before enrollment. The evaluation helps determine whether the program is appropriate and safe for the member. Enrollment, treatment, and medication prescribing are not guaranteed.

Why Specialty Programs Are Separate From Membership
Specialty add-on programs are billed separately from the base primary care membership because they require additional medical review, lab monitoring, medication management, safety screening, follow-up, and care coordination outside of routine primary care.
The base membership covers relationship-based primary care. Specialty add-on programs provide additional focused support for members who need more intensive monitoring or treatment planning in specific areas.
$99 Specialty Program Evaluation
A $99 Specialty Program Evaluation is required before enrollment in any specialty add-on program. The evaluation includes a provider review, medical history review, medication review, safety screening, and determination of whether the program is appropriate.
The evaluation does not guarantee program approval, medication prescribing, or enrollment into a specialty program.
Specialty programs may require a 3-month initial program agreement if approved.
