These Website Terms of Service, also referred to as "Terms," govern your access to and use of the website, online forms, enrollment links, contact forms, employer inquiry forms, training or event forms, and other online services operated by The Doc's Office Medical Group, PLLC, doing business as The Doc's Office – Advanced Health & Wellness and/or The Doc's Office, referred to in these Terms as "The Doc's Office," "Clinic," "we," "us," or "our."
By using this website or submitting information through our online forms, you agree to these Terms. If you do not agree to these Terms, please do not use this website.
1. Website Information Only
The information on this website is provided for general informational and educational purposes only.
Website content may include information about direct primary care, membership options, clinic services, employer plans, specialty programs, training opportunities, events, general health topics, and clinic updates.
Website content is not intended to be a substitute for professional medical advice, diagnosis, treatment, emergency care, or an established patient-provider relationship.
2. No Medical Advice From Website Use
Use of this website does not create a patient-provider relationship with The Doc's Office, Melissa Cahill, FNP-C, Dr. Brian Wasson, or any other provider, staff member, contractor, or representative of the Clinic.
Information on this website should not be used to diagnose, treat, cure, or prevent any disease or medical condition.
You should seek care from a qualified healthcare professional for medical questions, symptoms, diagnosis, treatment, medication decisions, or health concerns.
3. No Emergency Use
Do not use this website, contact forms, email, text messages, online forms, social media, or other general communication methods for emergencies, urgent medical concerns, severe symptoms, or time-sensitive medical needs.
For emergencies, call 911 or go to the nearest emergency room.
If you believe you may be experiencing chest pain, severe shortness of breath, stroke symptoms, major trauma, severe allergic reaction, suicidal thoughts, severe bleeding, loss of consciousness, or any life-threatening condition, call 911 or go to the nearest emergency room immediately.
4. Direct Primary Care Membership
The Doc's Office is a direct primary care practice. Information on this website about membership, pricing, services, employer plans, specialty programs, or other offerings is provided for general informational purposes and may change.
Website information does not replace the Direct Primary Care Membership Agreement, DPC Is Not Insurance Acknowledgment, Recurring Payment Authorization, Cancellation and Billing Policy, HIPAA Notice of Privacy Practices Acknowledgment, Patient Communication Consent and Acknowledgment, Notice of Privacy Practices, or other Clinic agreements and policies.
Enrollment, membership, and access to services are subject to acceptance by The Doc's Office, completion of required forms, payment terms, Clinic capacity, eligibility, scope of practice, and applicable Clinic policies.
5. DPC Is Not Insurance
Direct primary care membership with The Doc's Office is not health insurance, is not health or accident coverage, and is not a substitute for health insurance.
Membership does not cover emergency care, hospital care, specialist care, surgery, imaging charges, outside laboratory charges, prescription medication costs, or services provided by outside professionals, facilities, hospitals, pharmacies, laboratories, or specialists unless specifically stated in writing by The Doc's Office.
The Doc's Office recommends that individuals maintain health insurance, major medical coverage, catastrophic coverage, health sharing coverage, or another arrangement to help cover services not included in membership.
6. Enrollment
The Doc's Office accepts new patient enrollments through our Hint Health enrollment platform.
Submitting an enrollment form or providing payment information creates a membership agreement subject to the terms outlined in the membership agreement provided at the time of enrollment.
A patient-provider relationship is not established until The Doc's Office accepts the individual as a patient, required forms are completed, applicable payment terms are satisfied, the Clinic is open for services or otherwise ready to provide services, and the patient completes the Clinic's required establishment process.
7. Website Forms and Submitted Information
You may choose to submit information through website forms, contact forms, enrollment links, employer inquiry forms, event forms, or other online tools.
You agree that information you provide will be accurate, current, and complete to the best of your knowledge.
Please do not submit urgent medical concerns, detailed medical history, sensitive medical information, or emergency-related information through general website forms.
The Doc's Office may use submitted information to respond to inquiries, provide clinic information, process enrollment or membership-related requests, respond to employer inquiries, respond to training or event inquiries, and manage administrative workflows.
8. Privacy Policy and HIPAA Notice
Use of this website is also governed by The Doc's Office Website Privacy Policy.
Once The Doc's Office begins providing patient care, protected health information may be handled according to The Doc's Office Notice of Privacy Practices.
The Website Privacy Policy and Notice of Privacy Practices are separate documents and serve different purposes.
9. SMS, Text Messaging, and Email
If you provide your phone number or email address, The Doc's Office may contact you according to applicable law, your consent, the Website Privacy Policy, the Patient Communication Consent and Acknowledgment, and Clinic communication policies.
Message and data rates may apply for text messages. Message frequency may vary.
When supported by the messaging system, you may reply STOP to opt out of certain text messages or HELP for assistance.
Consent to receive promotional text messages is not a condition of purchase or membership.
Standard text messaging and email may not be fully secure. Do not use text, email, website forms, or online inquiries for emergencies.
10. Third-Party Platforms and Links
This website may link to third-party platforms, including but not limited to Hint Health, Stripe, Spruce, Elation, Elation Passport, social media platforms, payment processors, scheduling tools, maps, laboratory partners, pharmacy partners, or other third-party websites or services.
The Doc's Office does not control third-party websites or platforms and is not responsible for their content, privacy practices, security, terms, availability, or actions.
Your use of third-party websites or platforms is governed by their own terms, privacy policies, and rules.
11. Website Content and Intellectual Property
The content on this website, including text, graphics, logos, designs, images, service descriptions, documents, forms, and other materials, is owned by or licensed to The Doc's Office unless otherwise stated.
You may view, download, or print website materials for your personal, non-commercial use.

17. Employer Inquiries
Employers may submit inquiries through the website regarding employer-sponsored membership options.
Submitting an employer inquiry does not create an employer agreement, guarantee pricing, guarantee eligibility, or obligate The Doc's Office to provide services.
Employer-sponsored memberships are subject to separate employer agreements, eligibility rules, onboarding terms, billing arrangements, cancellation terms, Clinic capacity, and Clinic approval.
18. Training, CPR, First Aid, and Events
The Doc's Office may provide information about CPR, First Aid, training, community education, or other events.
Submitting an inquiry or form for a training or event does not guarantee registration, availability, pricing, certification, or acceptance unless confirmed by The Doc's Office in writing.
Event details, dates, times, locations, pricing, eligibility, and availability may change.
19. Promotions and Pricing
The Doc's Office may offer promotional pricing, employer onboarding promotions, or other limited offers.
Promotions may be changed, limited, extended, or discontinued at any time unless otherwise stated in writing.
Pricing and services listed on the website may change and may be subject to the applicable membership agreement, employer agreement, or Clinic policy.
20. Governing Law
These Terms are governed by the laws of the State of Texas, without regard to conflict-of-law rules.
Any disputes related to website use or these Terms will be handled according to applicable law and any applicable written agreement between you and The Doc's Office.
